The usual deadline is that you will need to submit the Assessment on Fitness to Drive Report and On-Road Assessment Checklist form to LTA at least 2 weeks before your upcoming
birthday.
However, if your birthday this year falls from 9 Sep to 6 Oct 2020, you may submit the Assessment on Fitness to Drive Report and the On-Road Assessment Checklist Form anytime from 2 months before your birthday to 2 weeks before 6 months after your birthday. However, you must submit the Assessment on Fitness to Drive Report and the On-Road Assessment Checklist Form to LTA and the Traffic Police (TP), if required, no later than 2 months after your medical examination.
For vocational drivers whose birthday this year falls before 9 Sep 2020, please check the letter from LTA on the different requirements you will need to fulfil.
