If I forgot to bring the Assessment on Fitness to Drive Report on the day of the On-Road Assessment, can I still proceed?

You can still proceed with the On-Road Assessment if the Driving Centre had verified or seen your Assessment on Fitness to Drive Report endorsed by a General Practitioner during registration and booking the appointment.


However, the Driving Centre will only endorse your On-Road Assessment Checklist Form when you return to the Driving Centre with both documents; Assessment on Fitness to Drive Report and the On-Road Assessment Checklist Form.


Once both the assessments on Fitness to Drive Report and the On-Road Assessment Checklist Form is endorsed by the Driving Centre; you may proceed to submit these documents separately to LTA (if you are turning 70, 73 and 74 years old) and Traffic Police (TP) (if you are turning 74 years old), via email at LTA_Assessment@lta.gov.sg and Traffic Police eService respectively.
