If I forgot to bring the Assessment on Fitness to Drive Report on the day of the On-Road Assessment, can I still proceed?

If you forgot to bring the Assessment on Fitness to Drive Report on the day of the On-Road

Assessment, you can still proceed with the On-Road Assessment if the Driving Centre had

earlier verified or seen your Assessment on Fitness to Drive Report endorsed by a General

Practitioner during registration. However, the Driving Centre will not endorse your On-Road Assessment Checklist Form immediately after you have completed the On-Road Assessment. You will need to return to the Driving Centre again with the Assessment on Fitness to Drive Report and the On-Road

Assessment Checklist Form for both forms to be endorsed. When both the assessments on

Fitness to Drive Report and the On-Road Assessment Checklist Form is endorsed; you may

then submit these documents separately to LTA (if you are turning 70, 73 and 74 years old) and Traffic Police (TP) (if you are turning 74 years old), via email at LTA_Assessment@lta.gov.sg and https://eservices.police.gov.sg, respectively.
